Respect the deadlines for both Sessions I and II.
Fill in your desired courses for Sessions I and II (please print legibly). Send them in together or you may observe the deadline given on each form.
Circle $ amounts or space (---) and verify your totals.
Verify your cheque. Incorrect amounts will not be accepted. You may risk not getting into the course of your choice.
Please DO NOT staple or scotch tape your cheque to the Registration Form(s).
$20.00 membership fee is due in September and is paid only once per year.
Keep a record of your cheque. (Cheques are not usually cashed immediately.)
No refunds will be issued for cancellations once courses begin.
Low registration can result in class cancellation. If you are interested in a course, please send your Registration Form and cheque as soon as possible.
For your own personal record, mark your chosen course(s) on your calendar and on the Course Index.
Verify your course starting date and room number.
